Visit our online AAGP Membership page to set up a profile and pay your initial annual dues.
Step #3
Login to the AAGP Website to access your dashboard. Update your account information with details about your practice, medical license, DEA license, and academic credentials. Please keep this profile current.
Step #4
E-mail copies of the following documentation for us to review using the email form here. You will receive an email confirmation of documents received within three business days. The board reviews applications once per month. Applicants are not eligible for the board certification program until the board has accepted their application.
- Your C.V.
- State Medical License
- DEA Certificate
- NPDB Self Query
- Proof of malpractice insurance
- UCSD PRR Certificate (if a graduate of their program)
- Previous AAGP Board Certificate (if previously a member)
Membership is provisional until all information is reviewed by AAGP leadership. A personal interview may be required. All decisions regarding membership in AAGP are final.
